Sauna has become wellness-industry shorthand for "detox" and vague good feelings. That framing undersells it. There's a real, two-decade body of cohort and clinical data on heat exposure and cardiovascular health, and it points somewhere specific: sauna use, done consistently and at a real dose, is one of the more evidence-backed lifestyle inputs we have for long-term cardiovascular risk. The problem is almost nobody talks about the dose. Frequency, temperature, and duration aren't details, they're the entire finding.
I think about this the same way I think about any system: the intervention isn't "use a sauna," it's "apply a controlled, recoverable stress to the body on a regular schedule." That's the same logic behind strength training and behind the interval work I've written about elsewhere. Heat is one more lever, not a separate wellness category.
What the data actually shows
The core evidence comes from a Finnish cohort of middle-aged men followed for roughly 20 years (Laukkanen and colleagues), later summarized in a 2018 Mayo Clinic Proceedings review and extended by a randomized trial on vascular function in patients with coronary artery disease. The finding that matters most: cardiovascular mortality tracked closely with how often people used the sauna, not just whether they did.
Compared with once-a-week use, cardiovascular death rates were meaningfully lower at 2 to 3 sessions per week, and lower again at 4 to 7 sessions per week, roughly a 70 to 75% reduction in cardiovascular mortality risk at the highest frequency after adjusting for other cardiovascular risk factors. Total weekly time in the sauna mattered too: more than 45 minutes a week was associated with substantially better outcomes than 15 minutes or less.
The people who benefit most in this data aren't doing anything exotic. They're doing something ordinary, at a real dose, for years.
One caveat worth stating plainly: this is cohort data. It shows a strong, consistent, dose-dependent association, not a randomized proof that sauna use alone extends life. The smaller randomized work on vascular function in cardiac patients supports a plausible mechanism, but it doesn't erase the limits of observational data. Treat the size of the effect with appropriate humility, and treat the direction of the finding as well-supported.
How often, how hot, how long
This is the part most sauna content skips, and it's the part that actually determines whether you get the benefit in the research or just a nice half hour.
Frequency: the strongest associations in the data show up at 4 to 7 sessions per week. That's a real commitment, not a spa day. If that's not realistic, 2 to 3 sessions per week still shows a measurable benefit over once-weekly use, it's just smaller. Fewer than that hasn't shown much signal either way.
Temperature: the traditional Finnish dry sauna protocol behind most of this research runs 80 to 100°C (176 to 212°F), measured at head height. That's meaningfully hotter than a lot of consumer "sauna" experiences marketed today. A mild, lukewarm room isn't applying the same stimulus.
Duration: sessions in the 15 to 20 minute range are where the benefit shows up most consistently; sessions under about 11 minutes had a weaker association. If you're new to heat exposure, build up to that over several weeks rather than sitting through a 20-minute session on day one. There's no bonus for gutting through dizziness or nausea, that's a sign to get out, not a sign of discipline.
Practically: aim for 4 to 7 sessions a week at 15 to 20 minutes each, in a dry sauna around 80 to 100°C, and build to that gradually. Hydrate well before and after. Don't combine sauna use with alcohol, it meaningfully raises the risk of the session and has been linked to acute cardiovascular events in sauna settings.
What's actually happening physiologically
Heat exposure at this dose is a hormetic stress, a controlled, recoverable strain that the body adapts to, similar in structure to exercise. Core temperature rises, heart rate climbs into a range comparable to moderate aerobic exercise, and the body upregulates heat shock proteins, molecules involved in repairing misfolded proteins and supporting cellular stress resilience. Regular sessions are also associated with improved vascular endothelial function, the kind of adaptation you'd expect to plausibly connect to the cardiovascular mortality data above.
What the evidence does not support
Sweating in a sauna is not a meaningful "detox" mechanism, the amount of heavy metals or toxins cleared through sweat is negligible compared to what the liver and kidneys already handle, regardless of what a lot of sauna marketing implies. Sauna use is also not a substitute for exercise, a whole-food diet, or sleep, it's additive to those, not a replacement for any of them. And nearly all of the strongest data comes from a Finnish population using traditional dry sauna. Whether the same dose-response holds identically for infrared saunas, wet saunas, or other populations is a reasonable question, not a settled one.
Who should check with a doctor first
Sauna use isn't risk-free for everyone. Pregnant women, anyone with a recent heart attack, unstable angina, severe aortic stenosis, significant low blood pressure, or a condition that impairs normal sweating should talk to their physician before starting a sauna practice, and anyone on medication that affects blood pressure or fluid balance should ask their prescriber whether it changes anything.
